Minimum Qualifications:
- Current and active CT Occupational Therapist License in good standing
- Graduation from an Occupational Therapy educational program
- Valid driver’s license in good standing and reliable transportation
- At least three years of experience in a clinical setting, preferably one year in home health care
- Good organization and communication skills
- Experience with electronic documentation/point of care systems a plus
- Previous home health care experience a plus
Duties and Responsibilities:
- The Occupational Therapist (OT) provides therapeutic services in the patient’s home setting.
- The services provided will follow a specific written plan of care established in coordination with a physician.
- Successful occupational therapists will need to have strong interpersonal and organizational skills, and be able to handle multiple tasks.
- The OT will work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care.
- OT’s provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.
- Instructs, motivates, and assists patients/caregivers in learning and improving activities of daily living
- Develops needed plans for modifying equipment and supportive activities to patients and family members.
- Supervises and directs the efforts of aides and Occupational Therapy Assistants, if one is assigned.

- Our therapists demonstrate timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of timesheets and other required patient/employment documents and reports.

What you will be doing:
1. After an initial visit has been made by the registered physical therapist or occupational therapist for evaluation of the patient and establishment of a patient care plan, the therapy assistant may provide ongoing therapy services in accordance with the established plan.
2. The therapy assistant, with prior approval of the registered physical therapist or occupational therapist, may adjust a specific treatment regime in accordance with changes in the patient’s status.
3. Prioritizes, plans treatment and travel times to provide for maximum efficiency, yet meet the needs of the patients.
4. Follows the Plan of Care and performs therapeutic interventions as directed by the Physical Therapist.
5. Completes and submits required documentation within established agency guidelines.
6. Monitors visit frequency and duration by reviewing physician’s’ orders and comparing to the Plan of Care and documentation.
What you will bring:
1. Successful completion of an education program accredited by the American Physical Therapy Association.
2. Minimum of One year experience as a Physical Therapy Assistant.
3. Successful completion of a Competency and Proficiency Test and Skills evaluation appropriate for the position upon hire.
4. Valid Physical Therapist Assistant license in the state of Connecticut.
5. Prior home care experience preferred.
6. Your own reliable transportation.

Summary:
Under the direction of a Registered Nurse (RN), the LPN visit nurse is responsible for providing routine skilled nursing care in the home, consistent with accepted nursing practice and agency policies. The LPN is responsible for keeping the RN, Supervisor of Clinical Services, and the attending physician informed on each patient’s status and progress. The LPN operates within the scope of practice defined by the State Nurse Practice Act. The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided by unlicensed staff.
Duties and Responsibilities:
- The LPN assists the RN in carrying out the patient’s plan of care.
- Observes conditions and reports changes in condition or family situation to RN
- Consults and seeks guidance from the RN as necessary
- Participates in the development, implementation, and continuing evaluation of the patient care plan.
- Participates in case conferences and staff consultation
- Participates in ongoing patient/family teaching
- Prepares, administers, and records medications per Agency policies
- Successful LPN visit nurses will need to have strong interpersonal and organizational skills, and be able to handle multiple tasks.

Description of duties:
As a Home Health Aide (HHA/CNA) you will be responsible for providing personal care and assistance with basic activities and daily living needs of patients. The Home Health Aide follows the home health aide instruction sheet and the directions of the supervising RN or Therapist. Examples of activities include: assistance with personal care such as bathing, transferring, dressing and undressing, ambulation, and selected homemaking and therapy services to our patients in their home or place of residence. The specific personal care for each patient will be determined by the registered nurse or therapist, and will be given in writing to the home health aide. Visits are scheduled within a territory, however visits may be made outside of the assigned territory as business demands.

Masters in Home Care is looking to fill a Full Time (in-office) Intake Manager position.
This position will oversee the Central Intake Department of 4 Full-Time Intake Coordinators, along with on-call staffing for coverage after hours.
This position will also fulfill the role of RN with other responsibilities, which is a state-mandated position required for licensure. The responsibility of this role is to participate in the competency compliance of the Home Health Aides hired by the agency.
This position requires an active RN license in the State of Connecticut, with at least 2 years of Home Care experience.
