Here's how you'll make an impact:

  • Responsible for the supervision of therapy practice team members and the oversight and maintenance of quality of patient care to meet federal, state and local regulations
  • Allocates and directs staffing needs to meet patient, unit/department and organizational needs and collaborates with others to ensure overall staffing needs are met
  • Manages performance process including: setting goals, clarifying job expectations, monitoring performance progress, providing feedback, recognizing performance, developing skills and addressing performance issues related to work
  • Participation in program planning, budgeting and evaluation activities to the clinical services of the Agency
  • Assures that all plans of care are accurate
  • Coordinates intake, admissions, referral and discharge process
  • Directs department staff meetings for dissemination of information to staff and to solicit input on the department direction/ongoing issues
  • Coordinates and participates in educational staff performance improvement in-services
  • Assists in the interviewing, hiring and separation of direct reports
  • Assists with orientation and in-service training for department staff
  • Participates in required clinical team conferences with other home health staff members
  • Provides therapy visits if need be

Qualities and experiences you will bring to our team?

  • Current licensure in the State of Connecticut as a Occupational Therapist
  • Minimum of three years of clinical experience
  • Minimum of one year in a supervisory role
  • Experience with electronic documentation/point of care systems a plus
  • Previous home health care experience a plus
  • Working knowledge of state and federal home health regulations
  • Excellent verbal and written ability
  • Valid Drivers License

Minimum Qualifications (where your foundation shines!):

  • You possess a current and active CT Physical Therapist License in good standing.
  • You have graduated from a Physical Therapy educational program accredited by the American Physical Therapy Association.

The Speech Language Therapist ensures quality and safe delivery of speech therapy and home health services rendered. Our SLPs coordinate provisions of home health care services that reflect the agency philosophy and standards of home health and speech therapy of assigned adult and geriatric patients.

What qualities and experiences will you bring to our team?

-Current and active CT Speech Therapist License in good standing

-Graduation from a Speech Language Pathology educational program

-Meet either of the following additional requirements:

  • The education and experience requirements for a Certificate of Clinical Competence in speech-language pathology granted by the American Speech-Language-Hearing Association
  • The educational requirements for certification and is in the process of accumulating the supervised experience required for certification

- Valid driver's license in good standing and reliable transportation

-Must have own transportation and be reliable

-Minimum of 1 year of clinical experience as a Speech Language Pathologist

-Flexible availability

-Good organization and communication skills

-Ability to follow and carry out instructions

-Experience with electronic documentation/point of care systems a plus

-Previous home health care experience a plus

Description of duties:

Evaluates, plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.
