Minimum Qualifications:
- Current and active CT Licensed Practical Nurse License in good standing
- Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements)
- 1 year of clinical nursing experience
- Valid driver’s license in good standing
- Must have own transportation and be reliable
- Flexible availability during the afternoon, evenings, and weekends.
- Good organization and communication skills
- Ability to follow and carry out instructions
- Experience with electronic documentation/point of care systems a plus
- Previous home care experience a plus
- Language: Spanish (required)
- Proof of satisfactory completion of a medication course a plus. (not required)
Summary:
Under the direction of a Registered Nurse (RN), the LPN visit nurse is responsible for providing routine skilled nursing care in the home, consistent with accepted nursing practice and agency policies. The LPN is responsible for keeping the RN, Supervisor of Clinical Services, and the attending physician informed on each patient’s status and progress. The LPN operates within the scope of practice defined by the State Nurse Practice Act. The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided by unlicensed staff.
Duties and Responsibilities:
- The LPN assists the RN in carrying out the patient's plan of care.
- Observes conditions and reports changes in condition or family situation to RN
- Consults and seeks guidance from the RN as necessary
- Participates in the development, implementation, and continuing evaluation of the patient care plan.
- Participates in case conferences and staff consultation
- Participates in ongoing patient/family teaching
- Prepares, administers, and records medications per Agency policies
- Successful LPN visit nurses will need to have strong interpersonal and organizational skills, and be able to handle multiple tasks.

What you will be doing:
- Providing outstanding nursing services to patients in accordance with a specific written plan of care established in coordination with a physician.
- Working on a clinical point of care system which provides real time access to patient data remotely.
- Enjoying an independent and flexible environment and having a significant impact on your patient’s lives.
What you will bring along:
- Current and active CT Registered Nurse License in good standing.
- Graduate of an accredited college or university.
- Valid driver’s license in good standing.
- 1 year of clinical nursing experience.
- 3 years of home health care nursing experience preferred.
- Strong med surg, acute experience.
- Working psych knowledge and/or experience
- Knowledgeable of nursing and medical practices and procedures
- Your own reliable transportation.

Masters in Home Care is looking to fill a Full Time (in-office) Intake Manager position.
This position will oversee the Central Intake Department of 4 Full-Time Intake Coordinators, along with on-call staffing for coverage after hours.
This position will also fulfill the role of RN with other responsibilities, which is a state-mandated position required for licensure. The responsibility of this role is to participate in the competency compliance of the Home Health Aides hired by the agency.
This position requires an active RN license in the State of Connecticut, with at least 2 years of Home Care experience.
