Masters in Home Care is seeking an experienced and mission-driven Utilization Review Manager (UR Manager) to oversee our Start of Care (SOC) nurses and support clinical excellence across all episodes of care. This position ensures efficient, clinically appropriate utilization of services, accurate and compliant documentation, and effective interdisciplinary coordination. The UR Manager plays a key role in maintaining operational efficiency, optimizing clinical outcomes, and fostering collaboration among nursing, therapy, and quality teams.

Key Responsibilities:

  • Supervise and support Start of Care (SOC) nurses to ensure consistency, compliance, and quality in clinical assessments.
  • Conduct utilization reviews to verify medical necessity, visit frequencies, and plan-of-care alignment.
  • Coordinate and develop disease-specific clinical pathways (e.g., wound care, chronic disease, post-acute transitions) and train staff on their implementation.
  • Lead interdisciplinary collaboration and case conferences to promote coordination of care and individualized treatment planning.
  • Monitor documentation quality, visit utilization, and medical supply costs to meet agency benchmarks and payer standards.
  • Partner with Quality Assurance and Clinical Education teams to identify training opportunities and implement process improvements.
  • Provide ongoing staff education and competency development sessions related to utilization, documentation, and clinical best practices.
  • Track and analyze utilization trends, including wound-care frequency, LUPA thresholds, and episode performance.
  • Communicate effectively with clinicians, physicians, and branch leadership to determine the best course of treatment for each individual patient.
  • Ensure regulatory and payer requirements are understood and followed by Start of Care nurses.

Qualifications:

  • Current Connecticut Registered Nurse (RN) license (required).
  • Bachelor’s degree in nursing or related field preferred; Master’s degree a plus.
  • Minimum of five (5) years of clinical experience in home health or a related healthcare setting, including at least two (2) years in utilization management, documentation oversight, or a supervisory role.
  • Demonstrated clinical expertise and ability to communicate effectively with other clinicians to guide individualized patient care decisions.
  • Strong understanding of Medicare Conditions of Participation (CoPs), Low Utilization Payment Adjustments (LUPAs), payer utilization requirements, and cost management strategies.
  • Clinical specialty certifications preferred (e.g., Wound Care Certification, Certified Case Manager (CCM/ACM), Utilization Review certification).
  • Excellent leadership, analytical, and communication skills.
  • Proven ability to manage utilization and control medical supply costs while maintaining high standards of care.
  • Proficiency in EMR systems and ability to interpret and act on clinical data reports.
  • Valid driver’s license and reliable transportation required.

What you will be doing:

  • Create and maintains staff schedules, including weekly productivity monitoring.

  • Schedule client appointments/visits into EMR according to care plans, expected case load, and staff availability.
  • Create and manage LPN/RN Team schedules.
  • Contact and communicate with staff and clients regarding day-to-day changes, call outs, missed visits, rescheduling, etc.
  • Enter scheduling data into EMR/creates schedules.
  • Work closely with the branch to prioritize scheduling needs based on direction from the clinical management team.
  • Communicate regularly with clinical leadership on staffing needs, clinical workforce, strategies for coverage, lack of resources, availability of clinicians, etc.
  • Participate in morning rounds for scheduling for the day.
  • Enter staff and client information into EMR as needed.
  • Maintain staff and client statistics and reports.

What you will bring along:

  • High School Diploma or equivalent
  • Experience with Healthcare scheduling preferred but not required
  • 2 years of home health care experience preferred
  • Strong knowledge of medical terminology

Description of duties:

As a Home Health Aide (HHA/CNA) you will be responsible for providing personal care and assistance with basic activities and daily living needs of patients. The Home Health Aide follows the home health aide instruction sheet and the directions of the supervising RN or Therapist. Examples of activities include: assistance with personal care such as bathing, transferring, dressing and undressing, ambulation, and selected homemaking and therapy services to our patients in their home or place of residence. The specific personal care for each patient will be determined by the registered nurse or therapist, and will be given in writing to the home health aide. Visits are scheduled within a territory, however visits may be made outside of the assigned territory as business demands.

The Speech Therapist ensures quality and safe delivery of speech therapy and home health services rendered. Coordinates provision of home health care services that reflect the agency philosophy and standards of home health and speech therapy of assigned adult and geriatric patients.

Minimum Qualifications:

  • Current and active CT Speech Therapist License in good standing
  • Graduation from an Speech Language Pathology educational program
  • Meet either of the following additional requirements:
  • The education and experience requirements for a Certificate of Clinical Competence in speech-language pathology granted by the American Speech-Language-Hearing Association
  • The educational requirements for certification and is in the process of accumulating the supervised experience required for certification
  • Valid driver’s license in good standing and reliable transportation
  • Minimum of 1 year of clinical experience as a Speech Language Pathologist
  • Must have own transportation and be reliable
  • Flexible availability
  • Good organization and communication skills
  • Ability to follow and carry out instructions
  • Experience with electronic documentation/point of care systems a plus
  • Previous home health care experience a plus

Description of duties:

Evaluates, plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.

Client Population: Young Adult to Geriatric

Type of Disorders: Our caring therapists evaluate voice, speech patterns, language and swallowing, and assist with the latest techniques for more effective communication, including voice, fluency disorders, head injury and stroke.

  • Masters In Home Care works on a clinical point of care system which provides real time access to patient data remotely in the field from a tablet computer. Our therapists demonstrate timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports. We’re looking for independent and ambitious therapy professionals who are looking to practice therapeutic services in a flexible environment.
