What You’ll Bring (Minimum Qualifications):
- Current and active CT Licensed Practical Nurse License in good standing.
- Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements).
- 1 year of clinical nursing experience.
- Valid driver’s license in good standing and reliable transportation.
- Good organization and communication skills.
- Ability to follow and carry out instructions.
- Bonus Points: Experience with electronic documentation/point of care systems, previous home care experience, or proof of satisfactory completion of a medication course.
Role and Job Duties:
Under the direction of a Registered Nurse (RN), you will provide routine skilled nursing care in the home, consistent with accepted nursing practice and agency policies. You’ll be responsible for keeping the RN, Supervisor of Clinical Services, and attending physician informed on each patient’s status and progress.
Key Responsibilities:
- Assisting the RN in carrying out the patient’s plan of care.
- Observing conditions and reporting changes in condition or family situation to the RN.
- Consulting and seeking guidance from the RN as necessary.
- Participating in the development, implementation, and continuing evaluation of the patient care plan.
- Participating in case conferences and staff consultation.
- Participating in ongoing patient/family teaching.
- Preparing, administering, and recording medications per Agency policies.

Minimum Qualifications:

Description of duties:
As a Home Health Aide (HHA/CNA) you will be responsible for providing personal care and assistance with basic activities and daily living needs of patients. The Home Health Aide follows the home health aide instruction sheet and the directions of the supervising RN or Therapist. Examples of activities include: assistance with personal care such as bathing, transferring, dressing and undressing, ambulation, and selected homemaking and therapy services to our patients in their home or place of residence. The specific personal care for each patient will be determined by the registered nurse or therapist, and will be given in writing to the home health aide. Visits are scheduled within a territory, however visits may be made outside of the assigned territory as business demands.
