Here's how you'll make an impact:
- After an initial visit has been made by the Registered Physical Therapist or Occupational Therapist for evaluation of the patient and establishment of a patient care plan, the therapy assistant may provide ongoing therapy services in accordance with the established plan.
- The Therapy Assistant, with prior approval of the Registered Physical Therapist or Occupational Therapist, may adjust a specific treatment regime in accordance with changes in the patient's status.
- Prioritizes, plans treatment and travel times to provide for maximum efficiency, yet meet the needs of the patients.
- Follows the Plan of Care and performs therapeutic interventions as directed by the Physical Therapist.
- Completes and submits required documentation within established agency guidelines.
- Monitors visit frequency and duration by reviewing physician's’ orders and comparing to the Plan of Care and documentation.
What qualities and experiences will you bring to our team?
- Successful completion of an education program accredited by the American Physical Therapy Association.
- Minimum of One year experience as a Physical Therapy Assistant.
- Successful completion of a Competency and Proficiency Test and Skills evaluation appropriate for the position upon hire.
- Valid Physical Therapist Assistant license in the state of Connecticut.
- Prior home care experience preferred.
- Your own reliable transportation.

Minimum Qualifications (where your foundation shines!):
- You possess a current and active CT Physical Therapist License in good standing.
- You have graduated from a Physical Therapy educational program accredited by the American Physical Therapy Association.
- You hold a valid driver's license in good standing and have reliable transportation.
- You bring at least three years of experience in a clinical setting (ideally one year in home health care, showcasing your developed skills).
- You demonstrate strong organization and communication skills (essential for effective patient care and teamwork).
- Experience with electronic documentation/point of care systems is a plus (for efficient and accurate record-keeping).
- Previous home health care experience is a plus
Job Responsibilities That Make the Difference:
As a Physical Therapist (PT) with Masters In Home Care, you'll deliver vital therapeutic services within the comfort of patients' homes. Your empathy will guide you as you implement a tailored plan of care developed collaboratively with a physician. Your analytical skills will help develop, evaluate and implement these plans, to address each patient's unique challenges.
Your attention to detail will ensure seamless coordination of care, maintaining communication with other healthcare professionals and identifying any additional interdisciplinary needs. Your enthusiasm will shine as you instruct, motivate, and support patients and their caregivers in improving their activities of daily living.
You may also have the opportunity to supervise and mentor Physical Therapy Assistants, fostering a collaborative team spirit.
Our clinical point-of-care system is highly efficient, providing real-time remote access to patient information via a tablet. This empowers you to demonstrate your commitment to timely and accurate documentation, facilitating the creation of OASIS data sets, service orders, timesheets, and other essential reports.
