Here’s how you’ll make an impact:

Assess psychosocial and financial needs of the patient to identify and implement a plan, using community resources and referrals, to assist the patient in maintaining self at home, counseling and end-of-life decisions. The Social Worker may assist with placement if patient unable to be managed in the home setting.

Minimum Qualifications:

• Graduation from a masters degree social work program accredited by the Council on Social Work Education
• Valid driver’s license in good standing
• Minimum of 3 years of experience as a social worker in a health care setting
• Must have own transportation and be reliable
• Flexible availability
• Good organization and communication skills
• Ability to follow and carry out instructions
• Experience with electronic documentation/point of care systems a plus
